Health insurance reimbursement

A personal trainer works with client on fitness equipment

Many insurance companies will reimburse you up to $20 each month for getting your workouts in.

Please contact your insurance provider for eligibility and workout requirements. Seasonal memberships do not qualify for health insurance reimbursements.

Sign-up for reimbursement 

What you will need

  • An annual community center membership (seasonal memberships do not qualify)
  • Your health insurance card
  • Banking information to set up direct deposit
  • Your Shoreview Community Center barcode number (located on your membership card)
  • Call 651-490-4760 if you don't know your membership code

Sign up online

  1. Go to MyFitnessRewards.org
  2. Go to "MEMBERS Options"
  3. Click "new enrollment"
  4. Select your insurance provider
  5. Enter your insurance group number, then enter our zip code: 55126 (change your radius to 5 miles)
  6. Find the Shoreview Community Center and click "enroll"
  7. Read and agree to the terms by clicking the box and then click "apply"
  8. Fill out the entire “Member Enrollment” form. This includes “Fitness Center Barcode"
  9. Submit your information. You will receive a confirmation email from My Fitness Rewards
