Health insurance reimbursement
Many insurance companies will reimburse you up to $20 each month for getting your workouts in.
Please contact your insurance provider for eligibility and workout requirements. Seasonal memberships do not qualify for health insurance reimbursements.
Sign-up for reimbursement
What you will need
- An annual community center membership (seasonal memberships do not qualify)
- Your health insurance card
- Banking information to set up direct deposit
- Your Shoreview Community Center barcode number (located on your membership card)
- Call 651-490-4760 if you don't know your membership code
Sign up online
- Go to MyFitnessRewards.org
- Go to "MEMBERS Options"
- Click "new enrollment"
- Select your insurance provider
- Enter your insurance group number, then enter our zip code: 55126 (change your radius to 5 miles)
- Find the Shoreview Community Center and click "enroll"
- Read and agree to the terms by clicking the box and then click "apply"
- Fill out the entire “Member Enrollment” form. This includes “Fitness Center Barcode"
- Submit your information. You will receive a confirmation email from My Fitness Rewards
