Eligible insurance companies

The Shoreview Community Center participates with the following insurance companies, which offer monthly reimbursements up to $20 toward qualifying annual memberships:

  • Prime West Health
  • Sanford Health

  • UCare

Please contact your insurance provider for eligibility and workout requirements. Seasonal memberships do not qualify for health insurance reimbursements.

What you will need

  • An annual community center membership (seasonal memberships do not qualify)
  • Your health insurance card
  • Banking information to set up direct deposit
  • Your Shoreview Community Center barcode number (located on your membership card)

Sign up online

  1. Please read your insurance information before enrolling in NIHCA Fitness Incentives
  2. Go to NIHCArewards.org
  3. Go to "Members Options"
  4. Click "first time enrollment"
  5. Select your insurance company
  6. Enter your insurance group number to see if you qualify, then enter our zip code: 55126 (Hint: change your radius to 5 miles)
  7. Find the Shoreview Community Center and click “enroll online”
  8. Read and agree to the terms by clicking the box and then click "apply"
  9. Fill out the entire “Member Enrollment” form. This includes “Fitness Center Barcode” (barcode on your membership card)
  10. Submit your information; you will receive a confirmation email from NIHCA Rewards

You can now log in to your NIHCA Rewards account to update billing and insurance information.
